Getting in contact
3 Ways To Get In Contact
  • Have your PCP send us a referral.
  • Submit a request via Healow through our website.
  • Call us at (480)462-3868 to provide patient and insurance information
Step 2
Insurance Verification
Insurance & Referrals
  • Our insurance team will verify your coverage and confirm if we're in network.
    They'll also determine any out-of-pocket costs.
  • If a referral is required, we'll coordinate with your child's PCP to obtain one.
  • Completing the ‘Brief Questionnaire’ while the insurance is being processed will help make sure we schedule your child appropriately.
Step 3
Payment and Scheduling
Scheduling Your Visit
  • Once insurance is verified and the referral is received, we'll schedule your child's appointment.
  • Payment is required at the time of scheduling, based on your insurance plan's specialist co-pay, co-insurance, or deductible.
Step 4
Questionnaires
Pre-Appointment Forms
  • Once your child's appointments are scheduled, you'll need to complete all required questionnaires for their assessment.
  • These questionnaires are essential and must be completed within 3 days of scheduling to avoid rescheduling.
Step 5
Appointments
Clinic Policy
  • Depending on your child’s age and needs they may require 3 to 4 appointments
  • Please note: To maintain testing accuracy, siblings or other children are not permitted in the clinic.
